Success Story: Philip W.

Philip W. came to Clearwater Nursing and Rehab on April 19th, 2018 after a long stay in the hospital. He was very weak and in need of therapy to build his strength back up and be able to return home. Philip was not able to transfer himself in and out of the wheelchair or propel himself around the facility. Philip also did not have insurance when he came to Clearwater. With the help of our business office manager and therapy staff we were able to evaluate and treat Philip. Therapy staff worked with Philip to develop a restorative nursing program to help build his strength. He was soon able to transfer himself in and out of his wheelchair. Philip enjoyed all the food while he was at Clearwater. This helped his body heal and develop muscles and strength to be able to return to his home! But, this is not where his story ends. Philip had one last road block in his way…having insurance benefits established to support him with medications and home health services upon his return to home. Luckily, he had a great team at Clearwater that was able to work on his Medicaid pending case for him. The business office manager spent time making several calls to follow up and ensure that all required documents were processed, the nursing staff worked with the doctor to get a letter explaining the need for a quick resolution, and our social service designee worked to coordinate home based services to ensure a smooth discharge for Philip.

On August 3rd, 2018 Philip Webber was able to return to his home! Philip Webber calls the facility to check in with our staff and make sure they know he is grateful for all the care and dedication the Clearwater staff provided him!
